We are seeking a skilled Licensed Practical Nurse to join our interdisciplinary team at Treasure Coast Hospice. As a key member of our clinical team, you will provide high-quality patient care and support to patients and their families in a compassionate and professional manner.
Key Responsibilities- Provide direct patient care and medication administration under the supervision of the RN Case Manager.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Conduct thorough assessments of patients' physical, psycho-social, spiritual, cultural, and biological needs.
- Utilize the nursing process to deliver clinical care services, including data collection, planning, implementation, and evaluation.
- Implement the nursing plan of care in accordance with physician orders and under the RN Case Manager's supervision.
- Provide pain and symptom management, including monitoring patients' responses to treatment and medications.
- Collaborate with other departments to develop program strategies and promote a comprehensive approach to patient care.
- Current and active LPN license in the state of Florida.
- Two years' experience in patient care, either in an acute care setting, long-term care, or home health agency.
- Hospice experience preferred.
- Knowledge of the nursing process, including data collection, planning, implementation, and evaluation.
- Computer literacy and experience with EMR systems, preferably Netsmart.
- Current and valid Florida driver's license with current auto insurance.
- Current BLS CPR certification.
