Nurse Practitioner

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Montgomery Nursing and Rehabilitation Center. As a valued member of our care team, you will provide daily support to our unit leadership and respond promptly to resident and family needs within the LPN scope of practice.
Key Responsibilities:- Use nursing process, judgment, and skills to set priorities and deliver quality direct resident care.
- Perform and administer standard nursing treatments and medications, observing residents for desired or unwanted effects.
- Communicate and collaborate with the care team to revise and update the Nursing Care Plan or any pertinent findings.
- Monitor and document vitals, treatments administered, and residents' responses.
- Prepare, organize, oversee, monitor, and evaluate the delivery of care for resident CNA assignments and activities.
- Graduate of an approved school of Nursing.
- Licensed and currently registered in good standing in New York as an LPN.
- Dedicated to providing extraordinary, clinically sound, and compassionate care.
- Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health.
- Have excellent communication, customer service, and organization skills.
- Work well under pressure and willing to work harmoniously with all staff.
