About the Position

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Travel Nurse in a Long Term Care (LTC) - Skilled Nursing Facility (SNF) setting in Alamosa, Colorado.

Job Summary

This is a Long Term Care RN position that requires a strong commitment to delivering exceptional patient care and making a positive impact in the healthcare community.

Responsibilities
  • Provide high-quality patient care in a LTC - SNF setting
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
Requirements
  • Current RN license in the state of Colorado
  • At least 2 years of recent experience as a RN in LTC - SNF
  • Current BLS and/or ACLS certification
  • 2 current clinical references
  • Physical and TB records
  • Current vaccinations (Tdap, Flu, etc.)
  • Titers - MMR/Hep B/Varicella or vaccination records
  • Ability to pass background/urine drug screening
