Job Summary

We are seeking a compassionate and skilled Personal Care Nurse to provide one-on-one care for medically fragile children in the comfort of their own homes.

About the Role

  • Implement physician-prescribed plans of care for patients
  • Assess patient needs and coordinate care with healthcare professionals
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document all care provided in our electronic system of record

Responsibilities

  • Health promotion and education for family members
  • Verbal orders from physicians, including obtaining co-signature from Case Manager
  • Follow up on patient problems, concerns, and changes in condition

Qualifications

  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test within the last 12 months
  • 6 months prior hands-on nursing experience preferred
  • Reliable transportation
