Key Responsibilities
The Patient Care Coordinator (Licensed Vocational Nurse, LVN) plays a crucial role in assisting the Patient Care Manager with the coordination of a dedicated team to ensure the delivery of high-quality care to home health patients.
- Facilitates the team's commitment to providing efficient, quality care in alignment with physician directives and State/Medicare regulations.
- Organizes patient lists and essential documentation for weekly case discussions.
- Delivers direct patient care as needed, adhering to the established scope of practice, care plans, and physician orders, while collaborating with other healthcare professionals.
- Supports the Patient Care Manager in reviewing clinician documentation and conducting audits to guarantee the completeness and accuracy of all submissions by field staff.
Licensure Requirements
- Current CPR certification is mandatory.
- Active LPN license in the state of practice.
- Valid Driver's License, vehicle insurance, and reliable transportation or access to public transit.
