This part-time position offers a competitive hourly rate of $18.00 and the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role
In this critical role, you will assist patients with daily living activities, maintain a safe environment, and collaborate with our interdisciplinary team to ensure comprehensive care. You will follow the Plan of Care established by the hospice nurse, participate in patient care coordination, and perform various healthcare procedures under supervision.
Key Responsibilities
- Assist patients with bathing, toileting, skin care, and ambulation
- Maintain a safe environment by ensuring proper use of equipment and preventing falls
- Collaborate with the hospice nurse to develop and implement individualized care plans
- Prepare meals and perform light housekeeping tasks as necessary
- Assist patients with exercises prescribed by appropriate professionals
Requirements
- Certification as a NYS Home Health Aide and annual continuing education
- Basic understanding of hospice philosophy and commitment to customer service
- Ability to work effectively in a team setting and maintain confidentiality
- Physical agility to perform repetitive tasks, including lifting greater than 50lbs
