Nurse Care Specialist

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Consulate Health Care. As an LPN, you will play a vital role in providing high-quality nursing care to our residents. Your responsibilities will include collaborating with our healthcare team to implement individualized treatment plans, supervising and evaluating Certified Nursing Assistants (CNAs), and overseeing fall-risk and pressure ulcer prevention programs.
Responsibilities
- Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.
- Collaborate with the healthcare team to develop and implement individualized care plans for each resident.
- Supervise and evaluate CNAs to ensure they are delivering care according to our high standards.
- Oversee fall-risk and pressure ulcer prevention programs to ensure the safety and well-being of our residents.
Requirements
- Active license to practice as an LPN.
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
