We are seeking a highly skilled Medical Care Professional to join our team at Abode Care Partners. This is an excellent opportunity for a compassionate and dedicated individual to provide exceptional care to seniors and patients with complex needs in various settings.
As a Medical Care Professional, you will work collaboratively with our interdisciplinary team to evaluate patient conditions, develop comprehensive care plans, and deliver evidence-based medical care. Responsibilities- Plan, direct, and evaluate patient medical needs based on evidence-based practices.
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues.
- Order and interpret laboratory and diagnostic testing as required.
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive care plans.
- Refer patients to other healthcare practitioners when necessary.
- Maintain up-to-date patient medical records.
- Holds an active state Nurse Practitioners license.
- Is currently certified as a Nurse Practitioner National Board.
- Has active state DEA licensure.
- Possesses a Master's degree in Nursing (MSN).
- Has five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Demonstrates strong interpersonal communication skills – verbal and written.
- Displays excellent time management skills with accountability for daily workflow.
- Shows proficiency in medical instruments and equipment.
- Has a strong knowledge base of HIPAA and OSHA regulations.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- Is a licensed driver with valid automobile insurance.
- Can engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day.
The estimated salary range for this position is $100,000-$120,000 per year, with total earnings potential reaching $130,000+ based on performance.
