Key Responsibilities
  • Attend and participate in staff meetings, educational programs, and community events to stay informed and network with colleagues.
  • Communicate identified needs and potential solutions to the Supervisor to ensure seamless collaboration.
  • Explain hospice services and benefits to patients, family members, and significant others in a clear and concise manner, fostering a positive view of hospice care.
  • Obtain signatures on all required forms and provide copies to patient/responsible party, ensuring timely completion and submission.
  • Ensure all available medical records are turned in to the Admission Coordinator, maintaining accurate and up-to-date patient information.
  • Establish and maintain significant professional relationships with community, hospital, and facility education representatives to drive growth and development.
  • RN additional responsibilities include:
    • Facilitating the admission process to and from hospital and long-term care-based referral sources, ensuring smooth transitions.
    • Assessing referred patients' clinical needs and disease status, conferring with patient's attending physician and hospice physician to establish eligibility.
    • Coordinating patient discharge from hospital to home, establishing initial needs and arranging transportation, DME, medication needs, supplies, and admission by a home hospice nurse.
    • Overseeing GIP patient admission process based on identified symptom management needs for that level of care.
    • Working closely with facilities and hospitals to identify, transition, and admit patients appropriately for general inpatient services.
    • Providing education to hospitals and facilities on the purpose and benefits of hospice care.
  • Performing other duties as assigned by the Supervisor.
Requirements
  • Associate or Bachelor's degree in Nursing highly preferred.
  • Two to three years of related experience required.
  • Hospital and/or long-term care clinical experience highly preferred.
  • Experience in healthcare marketing preferred.
Skills and Abilities
  • Mathematical skills: ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Language skills: ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Strong written and verbal communication skills, with the ability to articulate and embrace the hospice philosophy.
Certifications, Licenses, and Registrations
  • Active and unencumbered Registered Nurse license, or Licensed Practical/Vocational Nurse, in state(s) of employment required.
Physical Demands and Work Environment

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.

In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
