Northern Counties Health Care is seeking a highly organized and detail-oriented Registered Nurse (RN), Licensed Practical Nurse (LPN), or Medical Assistant (MA) to join our Concord Health Center team.

Key Responsibilities:
  • Triage patient calls, utilizing clinical knowledge to assess needs and prioritize care.
  • Conduct thorough patient intake interviews and assessments, documenting accurate and up-to-date information in our Electronic Medical Records (EMR) system.
  • Administer medications and vaccines, following established protocols and ensuring patient safety.
  • Perform routine laboratory tests and phlebotomy, maintaining accurate records and adhering to quality standards.
  • Assist providers with procedures and treatments, demonstrating technical proficiency and a commitment to patient comfort.

Requirements:
  • Graduate of an accredited RN, LPN, or MA program with a current Vermont license.

  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
