Homecare Physical Therapist Opportunity

We are seeking a compassionate Physical Therapist to provide home-based physical therapy services to our members. Our Therapist will contribute to improving mobility and enhancing the quality of life for our patients.

Responsibilities:
  • Travel to and from patients' homes to provide physical therapy as ordered by a physician in accordance with the designated plan of care.
  • Design treatment plans that achieve results through personalized, 1-on-1 individualized care.
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  • Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons.
  • Diagnose movement dysfunction by listening to patients and observing them as they move about.
  • Develop quality, personalized, evidence-based care intervention programs that improve outcomes.
  • Use hands-on therapy such as massage to ease patients' pain and provide stimulation to promote healing.
  • Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly.
  • Instructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery process.
Qualifications:
  • Current Physical Therapy Licensure in NY State.
  • Graduate of an accredited PT program.
  • Proven work experience as a physical therapist.
  • Intensive education and clinical expertise.
  • Ability to manage patients with different types of personalities.
  • Current knowledge of treatment practices.
  • Excellent interpersonal communication skills.

  • Competitive salary range: $113,000.00 - $127,000.00.
  • Travel required: Yes.
  • Travel percentage: 100%.
