Your Impact

As the RN Case Manager, you will play a vital role in assessing the impact of a terminal diagnosis on patients' physical, functional, psychological, and environmental needs and activities of daily living.

Main Duties and Responsibilities
  • Visit patients in their homes or facilities to provide care and support.
  • Provide education on medication, disease prognosis, and end-stage disease process.
  • Coordinate care and medications with physicians and APRNs, utilizing a team approach.
  • Utilize an interdisciplinary team approach to caring for patients and their families.
  • Provide routine wound care, recommendations, and assessments.
  • Demonstrate knowledge of drains, trach, and PEG tube care and maintenance.
  • Manage and implement patient care plans according to state regulations and communicate changes to physicians and team members.
  • Assess, monitor, document, and report patient health and condition using required documents and reports.
  • Utilize an Electronic Medical Records (EMR) System for charting.
  • Ability and willingness to travel throughout the Franklin County Service area.
Required Skills and Education
  • Active Florida RN license in good standing.
  • BSN preferred.
  • Minimum of 2 years of nursing experience required.
  • Hospice experience a plus.
  • EMR experience required.
  • Strong computer skills with a working knowledge of Microsoft Outlook and Teams.
