About the Role:
The successful candidate will be responsible for planning and coordinating clinical activities related to consumer services transitioning out of residential treatment. Key duties include conducting admissions, assessments, individual and family therapy sessions, and developing wellness and recovery plans.
Key Responsibilities:
Conduct admission and Biopsychosocial assessments using standardized tools.
Prepare and review Interpretive Summaries based on assessment information.
Collaborate with consumers to develop Wellness and Recovery Plans.
Conduct monthly reviews of Wellness and Recovery Plans with consumers.
Document all clinical services and activities in accordance with licensing, Medicaid, and Joint Commission standards.
Administer verbal reprimands and learning experiences as per agency policies and procedures.
Participate in discharge planning for consumers and families.
Write Progress Reports for assigned consumers as required.
Consult with Program Coordinator and Clinical Coordinator before terminating consumer treatment.
Provide referrals and linkage for ancillary services as needed.
Deliver individual and group therapy sessions as required.
