St. Edmond's Home for Children is seeking a dedicated and compassionate Full-time LPN/RN to join our care team. As a key member of our healthcare team, you will be responsible for providing exceptional care to our residents with intellectual and physical disabilities.
Responsibilities:- Develop and implement individualized care plans for each resident, in collaboration with the therapy team and supervisor.
- Supervise and delegate tasks to direct care staff, ensuring the highest level of care and attention to detail.
- Conduct regular assessments and evaluations of residents' conditions, updating care plans as needed.
- Collaborate with the primary care physician and director of nursing to ensure accurate and timely implementation of physician orders.
- Provide education and support to family members and staff on resident care and medical concerns.
- Participate in I.H.P. conferences and periodic reviews, ensuring continuity of care and resident well-being.
- High School Diploma or GED
- Current LPN or RN licensure in the State of Pennsylvania
- 1-3 years of nursing experience, with 1-3 years of pediatric experience preferred
- Previous experience working with physically disabled and developmentally disabled children, preferred
