Transitional Staff Nurse RN

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Transitional Staff Nurse RN. In this role, you will be responsible for coordinating the delivery of healthcare services to individuals with developmental disabilities who attend our Day Programs and live in community-based homes.
Key Responsibilities- Monitor and coordinate the delivery of preventive and routine healthcare services for individuals with developmental disabilities in both Day Program and residential settings.
- Supervise the adequate completion of delegated nursing tasks to direct care staff and LPNs.
- Provide and participate in Agency training.
- Coordinate and provide health services and training on those services for staff.
- Oversee delegated health-related tasks to Direct Care Staff and LPNs.
- Provide medication training and certification to direct care staff per regulations and Agency policy.
- Adhere to and ensure facility compliance with all physicians, NP, PA, etc. orders.
- Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke the responsibility of a Direct Care Staff or LPN to perform or complete any delegated task.
- Based upon the independent judgment of the RN, the RN as the authority to suspend and/or revoke medication certifications and medication administration of the direct care staff or LPN.
- Complete a physical health and development on individuals every 6 months in Day Programs.
- In coordination with Program Managers in Day Programs, ensure safeguards are updated as needed.
- Within each month, complete a monthly breast exam on female clients if specifically ordered at Day Program by PMD.
- Coordinate medical program needs with community services, medical providers, and other related agencies.
- Ensure adequate scheduling and completion of health-related exams, appointments, laboratory/diagnostic testing as ordered or required.
- Monitor medication administration in the facility.
- Complete a thorough Annual Medication review (Non-ICF) or ensure a pharmacist completes a Quarterly Medication Review (ICF).
- For ICFs, complete quarterly nursing reviews for each person receiving services.
- Within each year, update the Plans of Nursing Services and ensure review with staff.
- Within each month, complete a monthly breast exam on female clients, if ordered by a physician and/or provides training to individuals.
- Coordinate and provide health services/training for staff.
- Oversee delegated health-related tasks to Direct Care Staff and LPNs.
- Provide medication training and certification to direct care staff per regulations and Agency policy.
- Participate as an active member of the Interdisciplinary Team.
- Maintain compliance with all applicable State/Federal regulations and Agency policy and procedure.
- Maintain communication with LPN (as applicable), Program Manager, Program Administration, Service Coordinator, and other staff as necessary.
- Full-time residential RNs will follow a rotation schedule for emergency staff training on Saturdays and Sundays.
- Comply with all Agency policy and procedures.
- Current NYS license to practice as a Registered Nurse.
- NYS Driver's License that meets Agency policy.
- 1 year of related experience.
- Ability to meet physical requirements of the position when performing the following:
- Standing.
- Walking.
- Sitting.
- Bending.
- Stooping.
- Squatting.
- Kneeling.
- Climbing.
- Lifting requirement of 35 lbs.
- Ability to move, reposition, and place individuals in pieces of equipment.
- Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) as per Agency guidelines.
