Key Responsibilities:
- Conduct primary care evaluations to determine individual patient needs and develop personalized care plans.
- Administer patient medication reviews and provide written documentation of findings.
- Provide services in accordance with LifeSource site-specific protocols and standards.
- Establish and maintain open and positive communications with contract site staff, physicians, and patients.
- Be knowledgeable of and adhere to LifeSource standards, policies, and procedures.
Requirements:
- Masters degree in Nursing/Physician Assistant Studies from an accredited educational institution.
- Current and valid license to practice in North Carolina.
- Active Federal DEA or ability to process one.
- Knowledge of professional nursing theory, practices, and regulations.
