About the Job

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Hospice Care. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Job Summary

The Registered Nurse Case Manager will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care transitions. Responsibilities
  • Assess patient needs and develop individualized care plans
  • Coordinate with healthcare providers to ensure seamless care transitions
  • Provide education and support to patients and their families
  • Collaborate with interdisciplinary team members to ensure comprehensive care
  • Document patient progress and outcomes
Requirements

To be successful in this role, you will need:

  • Graduate of an accredited school of nursing
  • One to two years of recent acute care experience in an institutional setting
  • One to two years of recent experience in home care
  • Current licensure in the state and BLS Hands-On CPR certification
  • Bachelor's degree, with one year of hospice care experience preferred
