About the Role

We are seeking a highly skilled and compassionate Certified Home Health Aide to join our team at Vinfen. As a key member of our clinical staff, you will play a vital role in providing exceptional care and support to individuals with complex health needs.

Key Responsibilities
  • Assist individuals with Activities of Daily Living (ADLs) and provide personalized care to meet their unique needs.
  • Administer medication under the Medication Administration Plan (MAP) guidelines and ensure accurate documentation.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Provide education and support to individuals and their families on health-related topics and disease management.
  • Participate in quality improvement initiatives and contribute to the development of best practices in clinical care.
Requirements
  • Current Certified Home Health Aide Certification (CHHA) in Massachusetts
  • CPR certification within two weeks of hire
  • First Aid certification within two weeks of hire
  • MAP certification within 90 days of hire
  • Safety Care certification within 90 days of hire
