Job Title: Digital Health Worker
**Job Summary:**
We are seeking a skilled Digital Health Worker to join our team at OSF HealthCare. As a trusted member of the community, you will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
**Key Responsibilities:**
* Visit patients virtually to check on their well-being and address social determinants that may hinder their ability to effectively care for themselves
* Work with medical providers and primary care teams to improve patient care and outcomes
* Collaborate with other agencies to increase accessibility to health and social services
* Provide support to patients to enable them to remain at home and care for themselves
**Requirements:**
* High School/GED
* BLS certification within 6 months of hire
* Current driver's license with own reliable transportation
* Community Health Worker (CHW) certification required or obtained within 6 months of employment
* Excellent interpersonal and communication skills
**Preferred Qualifications:**
* Multilingual
* Knowledge of healthcare systems and outreach/networking skills with local agencies/community based organizations
