Job Title:

Home Health Care LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Home Health Care, Inc. As a home health care professional, you will provide high-quality patient care and support to clients in their homes.

About the Role:

Key Responsibilities:

  • Provide patient care according to client's care plan and needs
  • Perform wound care treatments and assist clients with medication management
  • Teach the client and/or family members about care and support skills
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team

Requirements:

  • Licensed as a Practical Nurse / LPN in the state of Minnesota
  • Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
  • Demonstrated ability to work autonomously
  • Prior Nursing experience in a TCU, Hospice, or Home Health setting
  • Experience with Electronic Medical Records (EMR) software is preferred
