St. David's Healthcare at Home is seeking a skilled and compassionate Home Health LVN to join our team. As a Home Health LVN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Responsibilities- Administer skilled nursing care to patients in their homes, under the direction of a Registered Nurse and/or Director of Professional Services.
- Conduct ongoing assessments to identify the physical, psychosocial, and environmental needs of patients.
- Develop and implement individualized plans of care, incorporating short and long-term goals, as established by the Plan of Care.
- Communicate effectively with patients, caregivers, and healthcare team members to ensure seamless care coordination.
- Provide education and support to patients and caregivers on disease management, treatment regimens, and medication administration.
- Graduate of an accredited LPN/LVN program.
- Minimum of one year experience as an LPN/LVN in a clinical care setting.
- Home care experience preferred.
- Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services.
- Meets all additional requirements for hire, including state-specific requirements.
- Current BCLS prior to delivering patient care.
- Reliable transportation and proof of valid automobile liability insurance.
