Job Summary:

The Registered Nurse - Home Health is responsible for delivering skilled nursing care to patients in their homes, while adhering to confidentiality standards and professional boundaries. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Develop and implement individualized care plans for patients, in collaboration with physicians and other healthcare professionals.
  • Provide education and training to patients and their families on disease management, medication administration, and other health-related topics.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide wound care, infusion therapy, and other specialized services as needed.
Requirements:
  • Active RN license in the state of application.
  • Valid CPR certification.
Preferred Qualifications:
  • Experience in skilled nursing or home health care.
  • Basic understanding of Oasis software.
