Morris Park Rehabilitation & Nursing Center is currently in search of dedicated Home Health Aides to fulfill the role of Patient Escorts and provide support to Nursing staff with non-clinical resident care overflow within our Skilled Nursing Facility.


Key Responsibilities:

  • Safely escort residents to and from their appointments.
  • Observe and monitor residents' well-being.
  • Report any health concerns to the supervising nurse.
  • Maintain confidentiality regarding all Resident and Facility information.
  • Assist in the collection and distribution of resident laundry.
  • Support family members in completing Patient Personal Property List Forms and deliver packages to resident rooms.
  • Assist residents during meal times.
  • Coordinate and aid in resident room changes, including the logging and packing of personal belongings.
  • Perform additional non-clinical resident care tasks as required.

Qualifications:

  • Must possess a valid New York State HHA Certification (available upon request).
  • Physical capability to maneuver a wheelchair.
  • Ability to stand for extended periods.
  • Capability to stoop or kneel as necessary.
  • Occasional heavy lifting may be required.
  • Ability to adhere to the Plan of Care accurately.
  • Must work effectively as part of a team.
  • Flexible availability is essential, including weekends and holidays.
  • Valid photo identification is required.
  • Must have undergone a physical examination within the last year.
  • Proof of PPD and immunizations within the last year.
  • Successful completion of a background check is mandatory.
