You will keep 100% of exam/professional fees and can opt into equipment/instrumentation and administrative support services for a fair market value. To be eligible for a lease agreement, you must have a Doctor of Optometry (OD) degree from an accredited optometry school, a valid state optometry license, and professional liability insurance. You will also need to provide a W-9 form and meet the requirements for ME & VA license agreements.
