Job Title: OB/GYN Social Worker

Job Summary:

The OB/GYN Social Worker plays a vital role in coordinating care management and behavioral health interventions for patients with complex care needs. Working in partnership with OB/GYN, Family Medicine, and Pediatric providers, this role ensures cost-effective, quality care outcomes for patients. The Social Worker coordinates care, develops strategies, and administers behavioral health screenings to ensure patients receive the best possible care.

Key Responsibilities:

  • Plan, implement, and coordinate care management services to optimize patient health.
  • Deliver brief interventions to support lifestyle and behavioral changes.
  • Integrate evidence-based clinical and preventative guidelines into individualized care plans.
  • Actively manage a caseload and provide targeted interventions to avoid hospitalization and emergency room visits.
  • Mobilize patients and significant others to make necessary lifestyle and behavioral changes.
  • Act as a patient advocate to ensure patient requests are recognized and followed.
  • Identify and respond to situations representing risk to patients or significant others.
  • Coordinate transition of care (TOC) calls and ensure medication reconciliation, follow-up appointments, and symptom assessment.
  • Develop and maintain a thorough knowledge of referral resources throughout the service area.
  • Maintain accurate and timely records, including clinical and progress notes.
  • Develop working relationships with community resource groups to coordinate care across the continuum.
  • Support education by supervising social work students for internships or other educational opportunities.
  • Act as a resource person for the healthcare team, providing in-service education and consultation on social and emotional factors related to health problems.
  • Maintain knowledge of the field, including Medicare, Medicaid, and other third-party reimbursement sources.
  • Participate in planning and implementing quality improvement activities.
  • Provide positive customer relations in dealing with patients, families, physicians, and other hospital departments.

Requirements:

  • Master's degree in social work from an accredited school of social work.
  • Current master's level clinical license with the State of Michigan or ability to obtain within six months of hire date.
  • Valid CPR certification.
  • Minimum three years of social work experience required, with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting.
  • Must have completed a certification in perinatal mental health through Post Partum Support International.
  • General computer knowledge and capability to use computer required.
