Job Summary

We are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team in the Lawton/Duncan region. As a primary care provider, you will be responsible for assessing patient health, creating care plans, and providing high-quality care to a diverse set of patients with chronic diseases.

Key Responsibilities
  • Assess patient health through physical examinations and medical histories
  • Create personalized care plans to address patient needs
  • Effectively communicate with office staff and clinical operations
  • Provide high-quality, patient-centered care to a panel of patients
  • Develop impressions, diagnoses, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications, supplies, and interventions as needed
Requirements
  • Board Certified with a valid NP or PA certification in the State of Oklahoma
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
