Job Title: Registered Nurse

Key Responsibilities:

  • Develop and implement individualized care plans to meet the unique needs of patients and their families.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support.
  • Provide education and guidance to patients and caregivers on health management and disease prevention.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.

Requirements:

  • Current licensure as a Registered Nurse in the state of practice.
  • Minimum 2 years of experience in hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
