About the Role

We are seeking a highly skilled Clinical Care Management Specialist to join our Complex Care team at Community Behavioral Health. As a key member of our team, you will play a vital role in providing intermediate or high-intensity care management services to members and their families.

Job Description

The successful candidate will be responsible for working directly with members, natural supports, and providers in the community to facilitate care coordination and address social determinants, barriers to wellness, and other needs. You will also be required to facilitate family Plan of Care meetings, complete utilization management activities, and authorize treatment when medical necessity criteria are met.

Key Responsibilities
  • Work directly with members, natural supports, and providers in the community to facilitate care coordination
  • Address social determinants, barriers to wellness, and other needs of members
  • Facilitate family Plan of Care meetings
  • Complete utilization management activities, including interagency meetings, social determinants interviews, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage, and authorization of treatment
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work or a related field
  • Clinical licensure (LSW, LPC, LMFT, LCSW) within 18 months of employment
  • Minimum 3 years of direct clinical care experience working with adults, children, adolescents, and/or their families in behavioral health settings
Preferred Skills and Qualifications

You will have excellent clinical case conceptualization skills, meeting facilitation skills, and the ability to work independently as part of a team. You should also be proficient in MS Office, especially Excel, and able to type at least 40 words per minute.
