Your RoleAs a CPN, you will play a vital role in partnering with the medical director to educate and engage hospitalist physicians and advanced practice providers in quality and process improvement efforts. Utilizing our proprietary metrics software, you will analyze key performance indicators to monitor and evaluate outcomes.

This multifaceted position within the hospitalist team does not involve direct patient care; however, you will engage in daily workflows, advocate for patient objectives within care plans, and ensure effective communication among the care team throughout the patient's hospital journey. Our commitment to quality healthcare extends beyond discharge, as we prioritize post-care support for our patients.
As a CPN, you will work a standard 40-hour week, Monday through Friday, in a fast-paced setting, while maintaining meticulous attention to detail and accuracy. Strong clinical knowledge and experience in an acute care hospital environment are essential, as is the ability to assess patient clinical status and readiness for discharge. Ideal candidates will possess:
- Current RN licensure in good standing; a Bachelor of Science in Nursing (BSN) is preferred.
- A minimum of two years of nursing experience; additional experience in case management, quality improvement, utilization review, transitional care, or post-acute services is advantageous but not mandatory.
- Familiarity with quality improvement initiatives related to patient satisfaction, length of stay, and readmission reduction is beneficial.
- Strong critical thinking skills and proficiency with technology, including electronic medical records and Microsoft Office.
