Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients' health needs and develop personalized care plans to ensure optimal outcomes.
- Skilled Nursing Treatments: Provide skilled nursing treatments as ordered by physicians, including vital sign monitoring, medication management, and wound care.
- Coordination and Collaboration: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Documentation and Record-Keeping: Maintain accurate and detailed records of patient care, including progress notes, medication lists, and vital signs.
- Patient Education and Support: Educate patients and their families on proper home health care procedures, strategies, and self-care techniques.
- Case Management: Identify and address potential health risks, develop strategies to prevent hospital readmissions, and provide ongoing support to patients and their families.
- Education and Licensure: Associate degree in nursing required; Bachelor's degree in nursing preferred. Current and valid California Registered Nurse License.
- Experience: Prior nursing experience, specifically in home health, preferred. Current CPR certification and driver's license required.
- Skills and Abilities: Strong interpersonal skills, computer literacy, and excellent written and communication skills. Ability to meet physical demands of the job, including lifting up to 50 pounds.
