Supervision: Consumer/Designated Representative
Objective: To deliver personalized support to Consumers in accordance with their Care Plan and directives from the Consumer/Designated Representative.
Key Duties:
- Follow the Care Plan meticulously, performing tasks such as dressing, grooming, personal hygiene, mobility assistance, toileting, light cleaning, and running errands.
- Adhere to the schedule set forth by the Consumer/Designated Representative.
- Ensure effective communication with the Consumer/Designated Representative.
- Abide by the Personal Assistant Agreement.
Qualifications:
- Requirements specific to the Consumer's needs as outlined in their Care Plan.
Additional Notes:
- Position may require lifting as necessary for Consumer support. Candidates must meet immunization and health assessment standards and complete mandatory training upon hiring and annually thereafter. Applicants must be at least 18 years of age.
