With a focus on personalized care, you'll work with a team of physicians and oversee LPNs, CNAs, and aides to ensure patients receive the best possible care. Your expertise will be valued and respected, and you'll have the autonomy to direct your own day.

As a Home Health RN Case Manager, your responsibilities will include:

  • Providing home-based care to patients with illnesses, injuries, and chronic diseases
  • Working with a team of physicians and overseeing LPNs, CNAs, and aides
  • Examining patients, assessing their progress, and reporting changes to their physician
  • Educating patients on their plan of care, medication, therapy, diet, and exercise
  • Coaching family members on the patient's plan of care and medication

Requirements for this role include:

  • A Registered Nurse (RN) license in the State of Michigan
  • At least one year of clinical experience as a Registered Nurse
  • Current CPR Certification
  • Reliable transportation with a valid state driver's license and current car insurance
