Job Summary

CVS Health is seeking a highly skilled Nurse Practitioner to join our National Virtual Primary Care team. As a key member of our care team, you will provide evidence-based, primary care to patients 18 years of age and older, working collaboratively with a dedicated care team to deliver high-quality, patient-centered care.

Responsibilities
  • Provide accurate and comprehensive assessments, diagnoses, treatments, and management of health problems for patients.
  • Develop and implement individualized care plans, taking into account patient preferences and values.
  • Collaborate with the care team to ensure seamless care coordination and communication.
  • Participate in daily care team huddles to review patient care, identify areas for improvement, and drive better outcomes.
  • Communicate effectively with patients, families, and healthcare professionals to ensure optimal care.
  • Document all patient care within our electronic health record, adhering to current policies and procedures.
Requirements
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing in at least one of the following states: Washington, Oregon, California, Idaho, Nevada, Arizona, Utah, Montana, Wyoming, Colorado, New Mexico, Alaska, Hawaii, North Dakota, South Dakota, Nebraska, Kansas, Oklahoma, Texas, Minnesota, Indiana, Missouri, Arkansas, Louisiana, Wisconsin, Illinois, Kentucky, Tennessee, Mississippi, Alabama.
  • Current National Board Certification, Family Practice.
  • A minimum of 5 years of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice.
  • Effective verbal, written, and electronic communication skills.
  • Outstanding organizational skills and ability to multi-task.
  • Initiative, problem-solving ability, adaptability, and flexibility.
  • Ability to work remotely in a care team culture.
  • Proficient with information management and technology.
  • Scheduled on-call rotation to provide after-hours care as needed for VPC members.
Working Environment
  • Virtual Care Team members must meet minimum requirements for remote care delivery, including: broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelines.
  • While performing the duties of the job, the employee is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses.
  • Specific vision abilities include the ability to view and read a computer screen and other electronic devices.
