Job Summary:

This position is responsible for providing interim and on-going care coordination services to families receiving services from Imua Family Services. You will work closely with families to identify their needs and develop personalized plans to support their child's development and well-being.

Key Responsibilities:
  • Provide family-centered culturally competent care coordination and social work services to children and their families
  • Develop and implement individualized plans to support child development and well-being
  • Collaborate with other professionals and community resources to ensure comprehensive support for families
  • Compile information and data for programming and accurate reports and billing
  • Provide information to other professionals and the community as needed
Requirements:
  • Bachelor's Degree in a related field from an accredited college/university
  • Licensed Social Worker/Ability to become LSW or LCSW preferred
  • Pediatric experience with birth to eight populations is preferred
