Dietitian

ChesPenn Health Services is a network of community health centers dedicated to providing quality, integrated, and culturally sensitive medical, dental, and behavioral healthcare to adults and children. Our mission is to create a healthcare home for every person living in the communities we serve.

We are seeking an experienced Dietitian to provide high-quality health education at our Federally Qualified Health Center in Upper Darby, PA. The successful candidate will work collaboratively with primary care providers to identify at-risk patients and design comprehensive group education programs to increase positive health outcomes.

• Work with primary care providers to identify patients in need of health education and design group programs to promote health and prevent disease
• Provide individualized health education and self-management support to patients
• Develop and implement health education materials and programs
• Participate in health education and promotion activities
• Maintain accurate records and submit monthly reports

• Master's Degree in Nutrition or related field
• Minimum of two years of experience as a Registered Dietitian
• Current COVID-19 and flu vaccines required
• Excellent organizational and interpersonal skills
• Proficiency in computer skills and Electronic Medical Records
