About the Role:
As a Post-Acute Care Navigator at Renown Health, you will play a vital role in ensuring seamless transitions for patients from acute care to post-acute settings. This position requires strong clinical expertise and effective communication skills to facilitate smooth referrals and coordinate care.
Key Responsibilities:
- Collaborate with interdisciplinary teams to identify patients eligible for post-acute care services
- Evaluate patient needs and develop individualized plans for care
- Communicate daily with case managers, physicians, and other stakeholders to ensure timely referrals and admissions
- Participate in marketing initiatives to promote post-acute care services and maintain consistent referral volumes
Requirements:
- 5+ years of clinical experience in an acute or post-acute setting
- Strong knowledge of CMS guidelines and post-acute care regulations
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
