**Job Summary**
SSM Health is seeking a highly skilled Family Practice/Primary Care Nurse Practitioner to join our team. As a Nurse Practitioner with SSM Health, you will have the opportunity to work with a diverse patient population and provide high-quality, patient-centered care.
**Responsibilities**
- Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough health assessments, develop and implement individualized care plans, and provide education to patients and their families.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between providers.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
**Requirements**
- Master's degree in Nursing or related field.
- Current licensure as a Registered Nurse and certification as a Family Nurse Practitioner.
- Experience working in a primary care setting, preferably in a hospital or health system.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
