We are seeking a skilled and compassionate Registered Nurse to join our team at Ascension. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.
Responsibilities- Provide direct nursing care to patients in accordance with established policies, procedures, and protocols.
- Implement and monitor patient care plans to ensure optimal outcomes.
- Monitor, record, and communicate patient condition as appropriate to ensure seamless care coordination.
- Serve as a primary coordinator of all disciplines to ensure well-coordinated patient care.
- Note and carry out physician and nursing orders to ensure timely and effective care.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
- Licensure / Certification / Registration: BLS Provider, American Heart Association or American Red Cross accepted; Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV); Licensed Registered Nurse credentialed from the Illinois Board of Nursing.
- Education: Diploma from an accredited school/college of nursing or Required professional licensure at time of hire.
- Additional Preferences: Home care experience a plus, background in acute or post acute settings.
