Job Summary:
As a Cardiac Critical Care Advanced Practice Provider at LifeBridge Health, you will play a vital role in managing critically ill patients post-procedure and requiring intensive care services. Collaborating with CV Intensivists and Attending physicians, you will provide continuity of care management until discharge. Your expertise will also support patient care for technical procedures, and you will work with the healthcare team to develop or modify plans for optimal, cost-effective, and high-quality patient care.
Responsibilities:
- Manage critically ill patients post-procedure and requiring intensive care services
- Provide continuity of care management until discharge
- Support patient care for technical procedures
- Collaborate with the healthcare team to develop or modify plans for optimal, cost-effective, and high-quality patient care
- Identify discharge planning needs for patients to ensure comprehensive discharge support
Requirements:
- Seasoned professional knowledge equivalent to a Master's degree
- Knowledge in more than one discipline
- Physician Assistant or Nurse Practitioner with acute care training
- 3-5 years of experience
- Active NCCPA Certification (PA) or Maryland CDS License and DEA License
- Active American Heart Association BLS/ACLS Certifications
- Active AANC or AANP Certification (NP)
