About the Role

We are seeking a skilled and compassionate Occupational Therapist to join our dynamic team at Infinity Rehab. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality therapeutic care to adults in their homes, including our assisted living communities.

Key Responsibilities

  • Evaluate and treat adults with a wide range of diagnoses and disabilities, with a focus on balance and fall prevention, therapeutic exercises for functional strengthening, patient and family education, mobility, core strengthening, and pain management.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals and family members to ensure comprehensive care and support.
  • Conduct home visits to assess patients' living environments and provide recommendations for modifications to promote safety and independence.
  • Stay up-to-date with the latest evidence-based research and best practices in occupational therapy.

Requirements

  • Bachelor of Science or Arts degree in Occupational Therapy from an accredited program.
  • Successful completion of National Certification Examination for Registered Occupational Therapist.
  • Current state licensure.
  • One year of home health experience as an OT (preferred).
