Job Title: Hospice Evening Nurse

At Andwell Health Partners, we are seeking a skilled and compassionate Hospice Evening Nurse to join our team.

Job Summary:

We are looking for a dedicated and experienced Hospice Evening Nurse to provide high-quality patient care in the comfort of their own homes. As a Hospice Evening Nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities:

  • Provide skilled nursing services to patients in their own homes, under the direction of a physician's plan of care.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of each patient.
  • Collaborate with the patient, family, and healthcare team to ensure seamless care coordination.
  • Assess and identify the patient's holistic health care needs, and develop strategies to address them.
  • Provide education and support to patients and their families to promote informed decision-making and self-advocacy.

Requirements:

  • Current R.N. license in the State of Maine.
  • ANA certification in Home Health or Community Health Nursing encouraged.
  • Current CPR card required.
  • One year of recent clinical nursing experience required.
  • A reliable car with insurance.
