We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a LPN, you will play a vital role in delivering patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Verify patient plans of care with the Registered Nurse (RN) prior to each visit.
  • Collaborate with physicians and RNs to ensure patient needs are being met, and notify them of any changes in patient condition.
  • Teach patients and caregivers self-care techniques to promote independence and well-being.
  • Check vital signs and review medications to ensure safe and effective care.
  • Perform specialized procedures as ordered by physicians, such as foley catheter changes.
  • Prepare equipment and materials for treatments using aseptic technique.
  • Complete documentation timely and accurately to ensure seamless care coordination.
Requirements
  • One year of clinical LPN experience.
  • Current LPN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
