Collaborative Care Model

We are seeking a highly skilled Youth Behavioral Healthcare Professional to join our team at Dartmouth Health. The successful candidate will work collaboratively with pediatric primary care providers, child psychiatrists, and other clinical staff to deliver comprehensive care for children and adolescents with behavioral health concerns using the Collaborative Care Model.

The ideal candidate will have experience working in or willingness to learn about the Collaborative Care Model and possess excellent communication skills. They will be responsible for conducting initial evaluations, providing psychoeducation, developing individualized care plans, and delivering evidence-based treatments.

Main Responsibilities

• Conduct initial evaluations and develop individualized care plans for patients
• Provide brief, evidence-based behavioral interventions such as behavioral activation, motivational interviewing, brief CBT, and parent skills training
• Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, monitoring side effects, and effectiveness of treatment
• Proactively communicate with patients, families, primary care providers, and the consulting child psychiatrist to deliver effective, efficient, and patient-centered care
• Facilitate connections to specialty mental health or substance use services when patients are in need of higher levels of care

Requirements

• Master's degree in Social Work, Mental Health Counseling from an accredited institution of higher education, including an internship meeting professional standards
• Must be able to demonstrate that they have provided psychological services, on an inpatient, outpatient or consultative basis in the past 12 months
• Licensed as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC)
