Position Overview:


The primary responsibility is to provide skilled nursing care to patients in their home environment, as directed by the physician and in alignment with the established care plan.

The LPN operates under the guidance of a Registered Nurse (RN) and collaborates with the interdisciplinary team, as well as the patient and their family.

The LPN will ensure care delivery adheres to State, Federal, and Medicare regulations.

Key Responsibilities:

  • Under RN supervision, assist in delivering nursing services and identifying the patient's physical, psychological, and environmental needs through observation, documentation, clinical records, team reports, and evaluations. Communicate findings to the assigned RN Case Manager.
  • Engage in planning, coordinating, and executing care in collaboration with the RN to ensure quality, cost-effective patient care.
  • Facilitate effective communication with patients/families, colleagues, and other healthcare professionals, as evidenced by clinical notes, case discussions, and evaluations. Accurately document the outcomes of interventions.
  • Support the patient/family in learning suitable self-care techniques and assess their responses to care.
  • Implement care while adhering to infection control protocols that safeguard both staff and patients (OSHA standards).
  • Assist the RN and/or physician in specialized tasks related to end-of-life care for hospice patients.
  • Support the admission, discharge, and transfer processes for patients.
  • May require on-call responsibilities.

Qualifications and Skills:

  • Demonstrated nursing skills in accordance with accepted standards of practice.
  • Proof of current Hepatitis consent/declination and TB vaccination records.
  • Proficient in using electronic medical charting and time-keeping software (Homecare Homebase).
  • Reliable transportation and valid auto liability insurance.
  • Ability to maintain professional boundaries in relationships with colleagues, patients, and caregivers.
  • Capable of identifying unsafe working conditions and promptly notifying a supervisor while attempting to rectify the situation safely.
  • Participation in annual in-service and performance improvement initiatives.
  • Fluency in reading, writing, and speaking English.
  • Consistent and reliable attendance.
  • Perform other related duties as assigned.

Required Skills:
Current Nursing License in the Practicing State
Valid Driver's License
Current CPR Certification
Minimum of two years experience as an LPN in a clinical setting, preferably in home health or hospice care.

Educational Background:
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
