Job Summary

At Medi Home Hospice, a part of Medical Services of America, we are seeking an experienced Full Time Registered Nurse to provide high-quality care to our Hospice patients in Clearfield and surrounding counties in PA.

About the Role

This is a unique opportunity to join our multidisciplinary team as a Registered Nurse, working under the general direction of the Director of Nursing or the Hospice Administrator. As a key member of our team, you will be responsible for providing support to patients and their families, maximizing comfort and health, and adhering to policies and procedures.

Key Responsibilities
  • Plan and provide care for patients, addressing their individual needs and collaborating with physicians to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to coordinate care and services.
  • Collaborate with multidisciplinary teams to develop and implement patient-centered care plans.
  • Assist with identifying patient/family needs and communicating with physician to ensure timely interventions.
Requirements
  • Bachelor's degree in Nursing from an accredited program.
  • Current licensure as a Registered Nurse in the state of PA.
  • Minimum one-year experience in direct patient care, preferably in hospice nursing.
  • Hospice nursing experience preferred but not required.
  • Reliable transportation and company required auto liability insurance.
