We are seeking a highly skilled Clinical Pharmacist to join our Inpatient team at Advocate Aurora Health. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications for our patients.

• Develop and implement individualized drug therapy regimens based on patient-specific factors
• Facilitate medication-related continuity of care at admission, transfer, and discharge
• Review and accurately interpret medication orders
• Develop competency in dispensing and compounding medications consistent with good pharmaceutical practice
• Ensure formulary adherence and recommend cost-effective therapies to providers
• Actively support procedures and initiatives to prevent medication errors

• Pharmacist license issued by the state in which the team member practices
• Two certifications: Immunization and non-vaccine injectable
• Bachelor's Degree in Pharmacy
• No experience required

This role requires the ability to stand for long periods, lift up to 35 lbs, and be exposed to chemical hazards. The ideal candidate will have strong communication and organization skills, as well as excellent presentation and training development skills.
