Key Responsibilities
  • Provide empathetic support to patients and their families
  • Educate families on available resources and programs
  • Visit patients in their homes and provide quality time with them and their families
  • Use advocacy, compassion, and problem-solving skills to create calmness in stressful situations
  • Maintain clinical records on all patients
  • Serve as liaison between patients and families/caregivers and community agencies
  • Maintain collaborative relationships with organization personnel to support patient care
  • Actively participate in quality assessment performance improvement teams and activities
  • Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
Requirements
  • Master's degree in social work, psychology, or sociology
  • Licensed Social Worker in the state of California
  • Prior hospice experience
  • At least one year of experience in healthcare
  • Understanding of hospice philosophy and issues of death/dying
  • Excellent verbal and written communication, and organization skills
