Key Responsibilities- Provide comprehensive nursing care to patients in ambulatory settings, including primary care clinics and community health centers.
- Collaborate with healthcare providers and support staff to develop and implement individualized care plans that address the unique needs and goals of each patient.
- Conduct thorough assessments, develop and implement care plans, and evaluate patient outcomes to ensure optimal health and wellness.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines and protocols.
- Stay up-to-date with the latest clinical research, best practices, and regulatory requirements to ensure the highest level of patient care.
- Graduate of an accredited school of nursing with a current RN licensure in the state of employment.
- 2+ years of experience as a Nurse in a primary care or ambulatory setting, with a strong background in patient-centered care and population health management.
- Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced, dynamic environment.
- Ability to maintain confidentiality and handle sensitive patient information with discretion and professionalism.
