Job Summary:

We are seeking a compassionate and skilled Caregiver to join our team in Suffolk. The successful candidate will provide high-quality care to our clients, working closely with our multidisciplinary team to deliver exceptional patient outcomes.

About the Role:

This is a full-time position requiring 16 hours per week, with flexible scheduling to accommodate your needs.

The ideal candidate will have a strong background in pediatric nursing, with experience in caring for children with complex medical conditions.

Responsibilities:
  • Implement physician-prescribed plans of care
  • Patient assessments and coordination of care
  • Administration of prescribed medication, treatments, and therapies
  • Compliant, timely documentation of care provided in system of record
  • Health promotion, teaching, and training of family members
Requirements:
  • Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Must have reliable transportation
