Facility and Location
GSS SD Sf Vlg is our facility located in Sioux Falls, SD. We offer a range of services, including general long-term care, short-stay rehab, and memory care.
Job Summary
The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN will demonstrate knowledge of nursing principles, competency, and practices within the full scope of nursing expertise/knowledge and utilize appropriate age and population-specific standards related to the physical and psychosocial needs of the resident as per care plan.
Responsibilities
- Ensures the health, comfort, and safety of residents.
- Contributes to the assessment of residents and administers medications or treatments as ordered.
- Provides technical support to healthcare professionals as needed.
- Documents resident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary.
- Counsels residents, family members, and/or resident advocate, under the direction of a registered nurse, advanced practice provider, or physician on prevention and treatment plans.
Qualifications
Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred. Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
